Unifying of medical records
The key goal of the Health Care Database is to unify all medical records of participating medical facilities country wide in order to "accelerate the seamless exchange of health information", The unification of records ensures that there are common nationally recognized standard to facilitate secure/confidential sharing and access to medical information for both the patients and medical personal across hospitals or clinics plus all their branches, this is refered to as interoperability. Further more, unifying of records significantly helps in offering efficient health care delivery while at the same time it helps in aggregating local content for medical research.

Homogenous treatment
Each and every patient in the Health Care Database has one integrated medical file across all participating medical facilities and their branches country wide, such an arrangement helps the doctor to know the treatment protocol a patient followed in the event of a hospital transfer (or if the patient has traveled to a new location) and thus ensure homogenous treatment.

Having all patients within a centralised Health Care Database means that, a patient being moved from hospital-A will be assured of getting the same treatment (and possibly medication) in hospital-B as the database system will enable a proper cordination for this to happen, not only does this guarantee accuracy in treatment protocols but also strengthens the referal chain and facilitates health insurance coverage.

Online registration
If you have access to the internet, you can register yourself or your medical facility into the database and also create your own profile right away. Alternatively; when you visit a participating medical facility, you will be asked to provide the respective profile details so that they can be entered into the database on your behalf.

After registration you get a unique Health Care Number and a Health Care Card that you can download and print on your own. When you are transferred to any other hospital, the unique health care number will help doctors there to recall your file without any delay, thus saving valuable time in case of an emergency.

Using the registration details, your electronic medical file (account) with a unique number is generated, every single medical record regarding you as the patient will (from that point on) be attached to the file electronically, this includes a history of medical conditions, lab reports, diagnosis, treatment protocols, dental records and any other medical history. Your medical file can be retrieved by authorised medical personal using your Health Care Number or your  NIN, alternatively your telephone number or your name can be used to search for your records depending on what is available to the doctor or what has been authorised by your next of kin.

Electronic Health Record (EHR/EMR)
The overall structure of the Health Care Database is based on the EHR model. An Electronic Health Record is a digital version of an individual's medical data. all data is collected and managed by multiple authorized healthcare providers, and can be exchanged electronically among them. It consists of information gathered by organizations over a period of time, it's often referred to as a longitudinal record rather than information from a single visit to a doctor or hospital.

An individual's health record may include demographic data, medical history and clinical information, such as laboratory, radiology and pharmacy data. Additionally, EHR systems give patients a way to access some portion of their electronic record through personal health record (PHR) services, which allow patients to view such information as lab results through a secure Web portal.

Personal Health Record (PHR)
The Health Care database gives you access to your personal health record, A personal health record is an electronic record that patients access with an application that allows them to manage their health information "in a private, secure and confidential environment", a PHR service is a component of EHR technology that gives patients a way to contribute information on symptoms or disease management to their records and communicate with their providers. A PHR contains the same information as an EHR but the patient can access it. ie a PHR is a subset of an EHR

Quick Response Code (QR Code)
We are currently using QR code technology at the back of every Health Care Card, this this technology allows quick responders to scan your card with any mobile phone that has a QR code reader in order to access vital information about you in case of an amergency. A QR code (abbreviated from Quick Response Code) consists of black squares arranged in a square grid on a white background, which can be read by an imaging device such as a camera, and processed using Reed–Solomon error correction until the image can be appropriately interpreted. The required data is then extracted from patterns that are present in both horizontal and vertical components of the image.

Hash Code Encryption (HCE)
We use HCE as an ideal way to store or transmit passwords and related data, hashes are inherently one-way in their nature, by storing passwords in hash format, it's very difficult for someone with access to the raw data to reverse it. With any future login attempts, we hash the password the user enters and compare it with the stored hash. If the two match up, then it's virtually certain that the user entering the password entered the right one. Hashing is great for usage in any instance where we want to compare a value with a stored value for security reasons.

Mobile Health (mHealth)

The Health Care Database is password protected, and therefore it does not display or release any part of your medical records without a login session by an authorised person or without your explicit consent and action. Every detail of any individual and device that accesses the database is captured and stored in the audit logs, the logs constantly record "which user did what" in the database and when, this ensures that "a track and trace procedure can be perfomed if need arises", the details captured may include: the protocal of the device (IP), device model or brand or version (IMEI), device ID (Serial No#), service provider (ISP), location (GPS), current user ID (HCN), date and time (GMT), browser ID, etc. The database automatically scans the audit logs to detect anomalies, such as an unfamiliar user or a known user logging on at an unusual time of the day/night.

Acredited medical facilities.
Your current doctor can only access your personal information if he/she has been authorised by an acredited medical facility, or you can use your own credetials to give temporary access to a doctor who is not part of the Health Care Database at the time when that doctor is attending to you. Further more, all log-ins (and viewed data) are recorded and reviewed by a participating medical facility in order to minimise the occurrence of unauthorised access.

Access by registred accounts.
Access to detailed records is only through a registred account, which may be authorized to access records for multiple individuals, e.g., so that a parent may manage records for their children, or a child may have Parental Guided (PG) access to their parent's records to help the parent deal with a cronic medical condition.

Role-based access.
The Health Care Database allows practices to configure their accounts to limit different levels of system access to employees who need to use a specific portion of the database and view the associated data. For example, a receptionist may only need to use the scheduling (queue) page; role-based access would not let that person access any clinical data which is reserved for medical personel or financial data which is reserved for accounting personal. This approach helps protect privacy and prevent the use of the database to commit fraud. In addition, if a user’s password is stolen and that person has only partial access to the database, it limits how much access any intruder can have. 

Masking Information.
The health care database protects privileged information with encryption, a method that masks or scrambles information so only authorized personnel can view the information in a patient record. In order to make the information “readable,” viewers must access the file with a personal encryption key or password.

Uptime Guarantee.
The database is currently accessible using two domains seated on two differrent servers that are both up and runing at the same time, if you fail to access through one of the domains then you can switch to the other domain or to the IP address of each domain, this four pronged access strategy is intended to guarantee 99.9% up time.

We use QR codes on all our Health Care Cards, by doing so, we have made the process of accessing your medical information much faster and easier during an emergency and when visiting the doctor. As a result, medical personnel are familiar with QR barcodes and know how to scan them using mobile phones.

All the above points clearly show that the Health Care Database provides univeral accessibility of vital health care infomation for doctors and medical facilities when it's needed in order to improve patient safety, quality of care and also to save life.

A very important point to note though is that the database is online, and so its access is restricted to internet connectivity, however internet connectivity is now available in 70% of the whole country, therefore most of the urbarn medical facilities should be able to easily access, while rural medical facilities can opt to use satellite based access.

NB: Doctors should not spend lots of critical time on trying to figure out what is wrong with you when such data is already accessible in your online profile, they must be able to examine your data directly from the Health Care Database and then quickly make informed decisions.

2 .1 .6 Who owns the data in the Health Care Dátabase?

Data stored within the Health Care Database is owned by two parties, ie the Medical facility that entered the data and also the patient whose data is all attached to his or her respective account.

Any of those two parties (for whatever reason) can request to have all their data exported and handed to them in Excel (CSV) formart for further processing or in tabular (html) format suitable for printing.

Therefore, the Health Care Database is simply a data store or a digital warehouse used to collect, collate, store, and disseminate data securely, its also acts as a data processing application which helps the two parties in retreaving and interpereting their row data, further more, the database helps the two partis to access thier data from anywhere when needed.

Its a fact that patients (before they fall sick) can create their own accounts online (or else they may need to consent to the creation of an account when they visit a medical facility), at that point, the initial bio data in the patient's account belongs to the patient.

However, medical facilities also enter prescriptions/diagnosis and accounting data against each patient at each visit, they also enter human resource data regarding the employees and medical personal who are handling the patients, at that point, such data also belongs to the medical facility which is generating it.

Its clear therfore, that the two parties jointly generate and thus own the data for which they can request a copy in the specified format. An important point to note though is that; several medical facilities can contriute data to the same patient's account as he or she roams around the numerous doctors and medical facilities, in that case the medical facility only owns a specific potion of the data that they contributed.

But, such a portion of fragmented data will not make much sense unless its attached (or related) to the patient's original bio details, thats why a request for a specific portion of data would also include the patients bio details which were not nessesarily entered by the requesting medical facility.

If you do not have a secure way to store or safe guard your own data then we advise that you do not make a request to extract it since its already secure and safer where it is.

The superadmin accounts

The superadmin account is used to controls access to the online administration of your whole organization's account and related top level privilages, this superadmin account also ensures that only legitimate representatives of an organization may act on its behalf since such accounts are the only ones allowed to do highlevel configurations and setting.​

To register a medical facility, you should complete the online regi​stration form by giving some basic details about the entire practice, The registration form asks for details of the Authorised Person and the Main Administrator for the entire account.

Interoperability

It provides access to life saving information.

If you were in an emergency situation and found unconscious, your Health Care Card will provide access to life-saving information to first responders, such as paramedics, firefighters, and police officers, as well as hospital personnel.

Your personal health record will be the same as the electronic health record that your doctor accesses, you will be able to store your most important health information and control who has access to it. you can even add your own information by scanning documents or typing in information. you thus have the option of adding information you think is relevant but that your doctor may not have, like information about your over-the-counter medications, exercise habits or sleep schedule.

Access to your Personal Health Record will increase engagement with all your doctors, which in turn, will improve health outcomes. All medical information entered by the doctor will be added to your account automatically, with the option of adding more information on your own. You will also have the ability to download your health information through your account.

HealthCareDatabase.net enables access to your medical records only at your discretion, for the sole purpose of providing you better healthcare especially in emergency situations.

Your medical history information is made available, by you, to your doctor, or any other interested party you designate to pull all of the information into one central file to be made available by you, only you, in order to assist the physician to quickly and accurately diagnose your current need.

While you are a member of the Health Care Database you may have the security and confidence that we will deliver the relevant information to your doctor, this will ensure that you get the proper care needed without delay or possible testing which has already been done by your primary physician.

Protection of your medical records is of paramount importance to us and thus the database system has numerous firewalls on its hosting platform to protect your information. We have made sure that there is no possible way that your information can be retrieved by anyone without your credentials.


3 .8 Terms of service

Every member of the Health Care Database has an online Medical Profile that contains biometric data or treatment infomation and existing conditions. It is the responsibility of the Account Holder to ensure that the account is updated by all the doctors and medical personnel who have been visited, its therefore advisable to visit only those medical facilities where your profile can be updated with your latest medical information so that you too can keep track of your health.

Sources of information

Multiple sources of information (eg: pharmacies, hospitals, Laboratories, clinic, etc) can be used to update a single Medical Profile. However, if the Account Holder fails to create or update the information within his/her own account then no information will be provided when the basic bio data is requested or when the Health Care Card (QR code) is scanned. The Account Holder has control over the Medical Profile and should ensure that it always has the basic information needed in case of an emergency.

Accurate Medical Profile

The Account Holder therefore accepts full and sole responsibility for maintaining a complete and accurate Medical Profile within his/her account. The Account Holder should also be aware that the information within the account my be updated by: doctors, pramedic, dentists, lab technicians, emergency reponders and many other entities after they have performed their duties, its the Account Holders responsibiity to log-in to the account and keep up to date with such information regarding his/her most current treatment and health.
 
It is solely the responsibility of the Account Holder to ensure that all information provided, including but not limited to medical history, medications, emergency contact names and contact information, is current and accurate. The Account Holder is also responsible for entering, maintaining and updating all information contained in the Medical Profile within the account.  The Account Holder is responsible for activating and deactivating all optional data within the account. 

Permission by the service provider

The Account Holder agrees not to reproduce, duplicate, copy, sell, resell or exploit any portion of the Service, use of the Service, or access to the Service or any contact on the website through which the service is provided, without express written permission by the service provider. We reserve the right to refuse service to anyone for any reason at any time. The Account Holder is solely responsible for all payments and all costs that may be associated with the service as offered by third parties like internet service providers.

Emergency Contacts 

The Account Holder grants permission to notify the designated Emergency Contacts of the time and location that an emergency has occured.  The Account Holder is solely responsible for designating emergency contacts, ensuring that designated emergency contacts have accepted this designation and providing complete and accurate emergency contact information.  We do not verify the accuracy of the contact information provided and will not check or confirm that the information entered is complete and accurate.  We will notify emergency contacts using the contact information that was provided by the Account Holder.  If the information is not correct, the emergency contact may not be notified. Only emergency contacts who have expressly accepted this designation will be contacted when the QR code on the Health Care Card is scanned.  We are in no way responsible for any party’s reliance or lack of reliance on an Medical Profile.

Changes to Terms of Service

We reserve the right to update, change or replace any part of these Terms of Service by posting updates and/or changes to our website. 4.0 Electronic Health Records in the Health Care Dátabase

The e-health menu is generaly restricted to medical personal only (based on their user rights), all items under this menu are designed to help administrators of medical facilities and medical staff to manage and control (or monitor) the entire medical practice and all its branches.  

In order to use the Health Care Database as a medical practitioner, you need to have your own peronal account first, If you have no account yet then you need to create an account right now, after which you can ask the Health Care Database adminsitrator of the medical facility (hospital/clinic) where you work to add you as a staff member.

However if your medical facility does not have an account in the Health Care Database yet, then you can ask the adminstrator of your medical facility to register and create an account right now.

When a person is checked into a hospital, the heathcare database will automaticaly send a notification to the next of kin, telling him of her about the situation.

Whether the person is sick, injured, or deceased, it is the additional responsibility of the hospital officials and local law enforcement officials to physicaly notify the persons next of kin. 

NB: When talking to the next of kin, its only doctors or medical examiners (coroners) that can declare an a person deceased, and only occupational medical staffs are authorized to discuss the medical condition of a peron in hospital if and when they have first hand knowledge of the same.
